Before you visit, make sure to fill out your paperwork and submit or bring with you along with your insurance card, ID, proof of income, list of any current medication you are taking and any questions you have. 

Unable to do the paperwork online, no worries, plan to check in 15-20 minutes prior to your appointment.

For the safety of your children and our staff, children under the age of 17 must be accompanied by an adult. Children under the age of 10,  CANNOT be left unaccompanied  on the premises, please plan accordingly.  ONLY the Parent or legal guardian and one child per family can be inside the clinic.

ONLY adults that need  special assistance can be accompanied by another adult.
